Parents/guardians and adult students generally must provide written consent before Personally Identifiable Information (PII) and education records (in digital or physical format) can be disclosed (unless an exception applies). A disclosure occurs when PII is shared in any way—whether orally, electronically, or in writing.
Consent forms must be signed, dated, specify who can receive records, which records can be disclosed, and the purpose of the disclosure
Consent can be provided electronically, if it meets the requirements above.
